Bunny Bunch Adoption Form and Questionnaire

Questionnaires may also be faxed to (909) 465-1352 or mailed to: P.O. Box 2583 Chino, CA 91708
Please feel free to ask us any questions you may have. We want you to make the right choice
in deciding to adopt a chinchilla. Please take your time to make the right decision. Many people
get a chin without realizing that it is a 16 to 20 year commitment. Chinchillas need lots of
care and attention from an adult. We are thankful that you are considering saving a chin!
